□ 한국의약품안전관리원(원장 한순영)은 국내 의약품 이상사례를 기반으로 개발한 의약품 안전정보(실마리정보*)가 세계보건기구 웁살라모니터링센터의 안전정보 공유 플랫폼(WHO-UMC VigiLyze)에 게재됐다고 밝혔다.
* 실마리정보(signal)란 약물과 이상사례 간 새로운 잠재적 인과관계 또는 알려진 관계의 새로운 측면을 제시하는 정보를 말한다.
○ WHO-UMC VigiLyze는 웁살라모니터링센터가 전 세계 회원국으로부터 보고받은 의약품 이상사례를 수집해 잠재적 안전정보(실마리정보) 등을 공유하기 위해 구축한 전산 플랫폼으로, 국내에서 개발한 실마리정보가 게재되기는 이번이 처음이다.
○ 이번에 게재된 실마리정보는 항악성종양제 젬시타빈 투여 후 파종혈관내응고 또는 미세혈관혈전증이 발생한 사례이다.
- 파종성혈관내응고 작은 혈관을 통해 응고인자 및 혈전용해효소(fibrinolytic enzyme)의 제어되지 않는 활성화에 따라 발생하는 출혈성 증후군
- 미세혈관혈전증 혈전성 혈소판감소성 자반(Thrombotic thrombocytopenic purpura) 또는 용혈성 요독 증후군(Hemolytic uremic syndrome)에서 발생하는 것처럼 작은 혈관 내 혈전 형성을 특징으로 하는 질환
○ 이 정보는 2019년 1월 국내 ‘젬시타빈’ 성분제제의 허가사항에 반영됐으며, 미국에서도 2019년 5월 ‘미세혈관혈전증’이 추가되었다.
□ 의약품안전관리원은 2016년부터 현재까지 국내 이상사례 보고자료를 기반으로 개발한 실마리정보 9건을 WHO 뉴스레터에 꾸준히 게재해 왔으나, 사례와 근거자료를 종합적으로 고찰하는 VigiLyze에 실린 것은 처음이다.
○ 이는 지금까지 VigiLyze에 실마리정보를 게재한 네덜란드, 에리트레아에 이어 세 번째 사례로 주목된다.
- 특히, 국내에서 개발한 실마리정보를 세계보건기구 국제약물모니터링프로그램(Programme for International Drug Monitoring)에 참여한 136개 회원국에 제공함으로써, 전 세계의 환자 안전과 정보 교류에 기여했다는 의미가 크다.
○ 의약품안전관리원은 앞으로도 국내에서 발굴한 새로운 안전정보를 알려 국제적인 의약품 안전관리에 적극 기여하겠다고 밝혔다.

Developed Safety Information on WHO-UMC VigiLyze
- KIDS Contributes to Global Safety Information Exchange and Patient Safety-
The Korea Institute of Drug Safety & Risk Management (hereafter KIDS, President Soonyoung Han) announced that a Signal Information*, detected based on domestic Adverse Event (AE) reports, has been published on WHO-UMC VigiLyze domain.
* A signal is information that arises from one or multiple sources, which suggest a new potentially causal association, or a new aspect of a known association, either adverse or beneficial, that is judged to be of sufficient likelihood to justify verificatory action. (CIOMS VIII(2010))
WHO-UMC VigiLyze is an online resource and global database tool developed by WHO-Uppsala Monitoring Center (WHO-UMC) to collect and manage Individual Case Safety Reports (ICSR) from member countries of the WHO Programme for International Drug Monitoring (PIDM). It serves as a platform for member countries to share possible safety information(signals) as well as current status of AE reporting in each nation; KIDS has published its signal information on this platform for the first time.
The article shared on VigiLyze Latest News contains signal information on administration of antineoplastic agent gemcitabine and subsequent occurrence of disseminated intravascular coagulation and thrombotic microangiopathy.
